Notification template
Under the Hazardous Substances (Classes 1 to 5 Controls) Regulations 2001, regulations 77, 83, 94, 101, 116 and 124 require the person in charge of a hazardous substance location or transit depot to notify an enforcement officer in the area where the site is located at least thirty working days (or five working days in the case of LPG sites) prior to commissioning the site.
In an effort to assist the person in charge to meet this obligation, and to ensure consistency with the information provided to the enforcement office, the following notification template should be used.
Instructions on the use of the template:
The person in charge should complete the fields in the template and then fax it to an enforcement officer stationed at one of the Department of Labour Branches nearest the hazardous substances location or transit depot. A copy of the completed notification template should be retained for future inspection or certification purposes.
Note: Should there be a significant change to the classes of hazardous substances or quantities held at the site, the notification should be revised to reflect these changes. The revised template should be sent to the Department of Labour.
